This session will explore practical, patient-centered approaches to deprescribing in frail older adults with limited life expectancy. Learning Objectives: (1) describe the principles of deprescribing in the context of frailty and limited life expectancy at the end of life; (2) identify common medication classes (e.g., antihypertensives, antidepressants, anticoagulants, hypoglycemics) that may be appropriate for deprescribing in patients with limited life expectancy; and (3) apply a structured, individualized approach to evaluating risks and benefits of continuing medications in end-of-life care.
