This session will present the latest peer-reviewed geriatrics research with questions and answers to follow. Learning Objectives: (1) discuss whether distinct forms of housing insecurity are associated with mortality and incidence of geriatric conditions among community-living older Americans; (2) review anticoagulant outcomes stratified by frailty level to inform clinical decision-making for older patients with acute VTE; (3) describe how a prognostic model can help guide shared decision-making and future planning between SNF clinicians and patients admitted to SNFs for post-acute care; (4) discuss a risk prediction model for older adults with polypharmacy or using high-risk medications to identify 1-year risk for hospitalization.
