This session will address two critical issues in the management of older adults: the overprescribing of certain potentially inappropriate medications and the under-prescribing of appropriate medications in historically underserved groups, including racial and ethnic minoritized populations. Learning Objectives: (1) identify disease/medication areas where there are important disparities in under-prescribing of evidence-based medications; (2) identify medication areas where there are important disparities in over-prescribing of inappropriate medications; (3) examine how the social determinants of health, including structural racism, may lead to under- and over-prescribing of medications; (4) identify strategies to effectively communicate medication optimization with minoritized older adults and their care partners.
